Aviv REIT, based in Chicago, specializes in the ownership and triple-net leasing of post-acute and long-term care skilled nursing facilities (SNFs). We have been in the business of investing in SNFs for over 30 years and own one of the largest portfolios of SNFs in the U.S. Our diversified portfolio consists of 357 properties in 31 states leased to 37 tenants. Aviv has developed a strong track record over 30 years as a reliable and relationship focused provider of capital. The Company has accomplished this with a management team that has an extensive knowledge of healthcare real estate.

Aviv's predecessor business was founded by Zev Karkomi, a well-known and respected real estate investor and a pioneer in triple-net leasing SNFs to nursing home operators. Mr. Karkomi was instrumental in cultivating many of our long-standing operator relationships and built Aviv into the significant platform that we enjoy today. Mr. Karkomi passed away in 2009 but his legacy continues to shape Aviv and drive our success.
